Testimonial, Likeness, and Media Release Waiver

This Release and Waiver (“Agreement”) is entered into by the undersigned individual (“Participant”) and BBALE-78 (“Company,” “we,” “us,” or “our”).

1. Grant of Permission

Participant voluntarily grants BBALE-78 the irrevocable right to use, reproduce, publish, distribute, display, and otherwise utilize the following materials provided by the Participant:

• Written testimonials

• Audio recordings

• Photographs

• Video recordings

• Statements, comments, or endorsements

This permission includes the Participant’s name, voice, image, likeness, and biographical information, in any medium now known or later developed.

2. Scope of Use

Participant agrees that these materials may be used for, but are not limited to:

• Website content

• Marketing and promotional materials

• Social media platforms

• Digital or print advertisements

• Educational or informational content

Use may occur without geographic limitation and for an unlimited duration, unless revoked in writing as outlined below.

3. Voluntary Participation

Participant confirms that:

• Their testimonial or media contribution is voluntary

• No financial compensation is required or expected (unless otherwise agreed in writing)

• Their statements reflect their personal experience and opinion

4. No Expectation of Privacy

Participant understands and agrees that:

• Once published, the materials may be publicly accessible

• BBALE-78 is not responsible for third-party use, sharing, or reposting of the materials beyond its control

5. Release of Liability

Participant hereby releases and forever discharges BBALE-78, its owners, operators, contractors, and affiliates from any and all claims, demands, causes of action, or liabilities arising out of or related to:

• Use of the Participant’s likeness, voice, or testimonial

• Claims of invasion of privacy

• Claims of defamation or misrepresentation

• Claims relating to publicity rights or intellectual property

This release applies to known and unknown claims, present and future.

6. No Endorsement Guarantee

Participant acknowledges that:

• BBALE-78 does not guarantee outcomes, results, or experiences for others

• Their testimonial does not constitute medical, legal, or professional advice

• Their experience may not be representative of all clients

7. Revocation (Optional, Goodwill Clause)

Participant may request removal of their testimonial or media by submitting a written request to BBALE-78.
BBALE-78 will make reasonable efforts to remove the materials from future use, but cannot guarantee removal from past distributions or third-party platforms.

8. Governing Law

This Agreement shall be governed by and interpreted under the laws of the State of Texas, without regard to conflict-of-law principles.

9. Acknowledgment and Consent

By signing below, Participant confirms that they:

• Have read and understood this Agreement

• Are at least 18 years of age

• Voluntarily agree to all terms without coercion
